How the Discovery That Australians Don’t Need a Gynecologist for Pap Smears Sparked a Major Discussion About America’s Healthcare Struggles

Recently, many Americans on TikTok expressed shock upon discovering that individuals in several countries have the option to conduct Pap smears themselves using a mail-order kit, allowing them to collect samples in their own homes.

The Australian government’s Department of Health provides detailed instructions indicating that the self-collection process is straightforward. Notably, the sample is collected from the vagina rather than the cervix, which differs from the conventional method performed during an in-office visit.

Their guidance notes, “All you need to do is insert a swab a few centimetres into your vagina and rotate it for 20 to 30 seconds.” This contrasts with the traditional Pap smear where a provider collects cells using a specialized tool, potentially resulting in a more uncomfortable experience.

While this method appears simple and convenient for residents of Australia, Canada, British Columbia, Denmark, the Netherlands, and Sweden, it remains unavailable to women in the United States. To understand the barriers to accessing at-home Pap smears, BuzzFeed consulted Dr. Marieme Mbaye, an expert in the field.

Dr. Mbaye explained that Pap smears serve as a critical screening tool for identifying abnormal cervical cells that could lead to cervical cancer. “Pap smears are essential because they can detect changes early on, before they develop into cancer,” she stated. The procedure includes examining cervical cells microscopically and conducting an RNA test for HPV, the virus responsible for most cervical cancer cases.

Interestingly, labs have advanced to the point where they can analyze samples collected from the vaginal wall, but the current lack of widespread acceptance for at-home Pap smears in the U.S. stems from ongoing studies about self-collected HPV tests, as noted by Dr. Mbaye. She indicated that while these tests could benefit individuals who face challenges accessing care, in-clinic Pap smears remain the most reliable and recommended practice for the time being.

“Most individuals should begin getting Pap smears around age 21 and continue every three to five years until age 65, provided results are normal,” Dr. Mbaye advised. She described how in-office exams involve gently inserting a speculum into the vagina to access the cervix, which resembles a small bagel with a central opening. The collection of cells is quick and ideally takes fewer than 30 seconds. While the procedure may feel awkward or uncomfortable for some, it should not be painful.

For those who may feel anxious about undergoing a Pap smear, Dr. Mbaye encouraged patients to communicate their concerns with their healthcare provider. “If it makes you uneasy, consider bringing a support person or discussing adjustments that could enhance your comfort during the procedure. Some facilities offer chaperones to assist during the exam, and smaller speculums are available upon request,” she said. “It’s important to advocate for yourself throughout the process.”

Under the Affordable Care Act, most private insurance plans are mandated to fully cover annual Pap smears and HPV testing without any out-of-pocket costs.
