Exploring New Treatments for Cutaneous Lupus Erythematosus

Systemic lupus erythematosus, commonly referred to as lupus, is a complex autoimmune condition that manifests with a range of symptoms and presents ongoing challenges in treatment approaches.

One prevalent form of this illness is cutaneous lupus erythematosus, which is particularly marked by rashes that can develop on various body areas, including the face and scalp, often resulting in hair loss and skin scarring.

The rashes associated with cutaneous lupus are a direct result of inflammation caused by the body’s immune system mistakenly attacking its tissues.

Typically, the treatment regimen for cutaneous lupus erythematosus involves the use of immunosuppressive medications and biologic therapies aimed at curbing inflammation.

Despite the efficacy of these traditional medications, many patients managing systemic lupus erythematosus find themselves on a complex array of medications and are actively seeking alternative treatments that can reduce reliance on pills.

Dr. J. Michelle Kahlenberg, a professor of internal medicine specializing in rheumatology at the University of Michigan Health, led a research initiative examining an innovative alternative—a topical treatment known as mupirocin.

This investigation stemmed from Dr. Kahlenberg’s earlier findings, which indicated that cutaneous lupus rashes frequently harbor Staphylococcus aureus, a common skin bacterium that exacerbates the inflammation within these rashes.

Mupirocin serves to eliminate this particular bacterium from the skin.

The study involved a randomized selection of patients with systemic lupus erythematosus who were suffering from active cutaneous lupus erythematosus flare-ups. Participants were treated either with mupirocin or with an inert control substance, petrolatum jelly.

Before and after the treatment, researchers collected samples from the nasal passages and the affected skin to assess baseline and post-treatment levels of Staphylococcus and the overall microbial community profiles.

Analysis of paired samples taken prior to the application of the topical treatment and seven days post-application revealed a significant reduction in Staphylococcus aureus in those treated with mupirocin.

Critically, this decrease in bacterial presence was also associated with reductions in key inflammatory signals, including those linked to interferon-driven gene expression within the lesions.

“Not only does mupirocin reduce inflammation by lessening the presence of lesional Staphylococcus aureus, but it also resulted in decreased levels of monocytes in the skin, which are pivotal in the pathogenesis of cutaneous lupus,” stated Dr. Kahlenberg.

Although mupirocin is currently a prescription treatment, and initial findings indicate potential benefits in reducing inflammation, the study was not structured to determine its effectiveness in directly alleviating the visible rashes of cutaneous lupus erythematosus.

“Larger and more comprehensive studies are needed to assess whether topical antibiotics can effectively eliminate rashes,” Kahlenberg emphasized.

“Nevertheless, this research represents an exciting preliminary step towards identifying additional treatment modalities that may help in managing inflammation beyond the conventional immunosuppressants and biologic therapies.”
